Each and every time you see the doctor, do not leave the doctor’s office without first receiving a note, signed by the doctor, stating whether you are temporarily disabled, released to light duty, or released to full duty employment. Without some form of a disability slip signed by the doctor, you will be unable to claim any wage benefits under the Virginia Workers’ Compensation Act. You cannot forget to ask for a disability slip at each appointment. The physicians should be aware of your need for a disability slip, so don’t feel bad about asking for one.
